ACCESS - Patient

Notes:

• Patient Access to their Portal requires a unique Username and Password.
• There are two Patient entry points to their Patient Portal: emailed Link and website URL.

Patient Portal Email Link - First-Time User:

1. Select the “Patients” area in the green, left-hand side menu.
2. This opens the “Patient List”.
3. Locate the Patient to be emailed using the search bar or manual scrolling.
4. Select the desired Patient from the "Patient List".
5. This opens the Patient’s "Patient Dashboard" page.
6. Select the blue “Emails & Texts” dropdown menu button in the “Patient Details” section.
7. Select “Email - Portal Link”.
8. This automatically emails the Portal Link with the following message:

Hello (Subordinate Patient's Name): The below link provides access to your personal Patient Portal. Please note that upon arrival, you will be asked to provide a Username and Password. Click the below link to get started. Patient Portal Thank you!

Patient Portal Website URL - Repeat User:

• After the initial visit and creation of login credentials, Patients access their Portal via the website URL www.vyapatient.com using their existing Username and Password.
